Previous breast surgery. Rather than mastectomies, nowadays we tend to see evidence of wide local excision on chest radiographs of patients who have had surgery for breast cancer. The breast shadows are asymmetric, and you will often see metal clips (arrow) at the site of surgery. It is important to recognize this finding, as it should prompt a search for evidence of metastatic disease.
