Acute infarct. This 19 year old patient was found unconscious and brought to the ED. CT showed extensive low attenuation in the left MCA and PCA territories, with loss of normal grey-white matter differentiation. This devastating infarct was subsequently found to be embolic in origin; the patient had a patent foramen ovale and the embolism had originated in a lower limb DVT.
